How to Use Your Fall Risk Prevention and Social Isolation Benefits

For members who at risk for Fall Risk, you may be eligible for a strength and balance training to improve physical activity and prevent falls. Programs are evidence-based or of similar duration and quality compared to evidence-based programs.

Members cannot join a program if they are non-ambulatory, have significant physical activity limitations that would present a safety hazard for participation in a community-based program, or if they are at very high risk of falling based on a structured fall risk assessment. In those cases, members should consult with their primary care physician and receive a multifactorial falls risk assessment and referral to community or home-based physical therapy, as appropriate.

Social isolation and loneliness is a concern for many of our Medicare Advantage members. Social isolation and loneliness can lead to higher risk for a variety of physical and mental conditions. Members may be eligible for programs that can connect individuals to social clubs, programs and people in the community to support mental and social wellbeing and reduce social isolation. Programs include those that connect members to community clubs and groups, and/or provide individual companionship in your home to help with your daily tasks or simply spend time together.
